This achievement/trophy is available in Act II, The Handy Man. All you need to do is kill a bunch of Ridden with a rather bizarre weapon.

Make your way through the level until you have to clear several Ridden nests. After you have destroyed the final nest, make sure you inspect the remnants and you will discover Bob’s Arm. This is quite literally the arm of a former handyman, and you can use it to smash zombies over their heads.

From here, simply equip the weapon and kill 10 Ridden with it, after which the achievement will pop and you’ll have completed it.
