First, if you haven’t actually yet started the mission, you’ll need to activate the side quest, which is undertaken by speaking with Ellie at the Sanctuary.

The quest reads:

“Put a stop to the COV’s auditions for a new mouthpiece!”

From here, fast travel to Ascension Buff in Pandora and start working your way through the mission objectives: Steal the Hammerclan Banner, Get Trophy from War Pig, Loot Puttergunk’s Strongbox.

The mission then culminates at the Holy Broadcast Center as you bring the trophies to that location.

Now that you have deposited the trophies and convinced the Gatekeeper to let you through, it’s just a question of shooting your way through the bandits and finally the Mouthpiece himself.

After dealing with him, seek out his organ. This is the key to unlocking a door that will take you to the secret chest you’re looking for.

As soon as you have interacted with the organ, look to your left and notice the broadcast room door is now unlocked. Run through the door and follow the corridor until you reach the chest at the end.

And that should be everything you need to know about where the Mouthpiece secret chest room location is in Borderlands 3.
